Check off sheet for 2008-2009

The following needs to be turned in during tryouts.

_____ Player’s original birth certificate (you will get the original birth certificate back)

_____ A copy of player’s birth certificate (If you were in WVJ last year, you do not need to

bring a copy of your birth certificate)

_____ 2009 individual Membership form signed (MUST sign the front and back)

_____ 2009 Player Medical Release Form signed

_____ A copy of the player’s insurance card

_____ West Valley Payment Agreement signed

_____ West Valley Absence Policy signed

_____ West Valley Juniors Release and waiver signed

_____ West Valley Parent Agreement signed

_____ $100 tryout fee which ($50 is non-refundable) make checks out to West Valley Juniors VBC
