14’s Payment Agreement 2008-‘09
For the participation of ______________________________ (player) in the West Valley Juniors Volleyball Club 2008/2009 program, I ________________________ (parent/guardian) jointly do hereby promise to pay to West Valley Juniors Volleyball Club the amount of $500.00. Should a team wish to participate in an extra tournament(s) the additional cost will be divided among the team members (A player will be charge additional $10.00 for each miss tournament in which she does not notify her coach at least 2 days prior to missing that tournament day). The above said amount is to be payable in installments as follows and any additional cost before the player participates in said tournament:

We will be ordering uniforms on 12/8/08. If a player wishes to order warm-ups (which is not required and it is an additional cost to the player), a parental permission slip will be required as well as a $35.00 deposit before 12/15/08.
An installment shall be considered late if it is not paid within seven (7) days of the due date and parent/guardian will be considered to be in default. In case of such default, the above named player will not be allowed to participate in any Club activities including, but not limited to, practice sessions and tournament, until the default is corrected. Since this is a team sport and it affects the whole team, if a player drops from a team the above player will be responsible for the entire cost of the season. If a player drops from the team, the participant will not be awarded any refund from the club. In addition, in case of such default the undersigned agrees to pay all reasonable expenses, including court costs and attorney fees, incurred during collection proceedings. A $25.00 fee will be accessed for any returned checks.
